Whether you have already left hospital after your operation, or would like us to participate in planning your care needs before admission, our qualified, friendly experts are here to help.

With our clinical nurse managers at your disposal, our Hospital to Home programme provides you with a service that is tailored exactly to your needs. This may mean a daily short visit or round-the-clock, complex care for specific conditions, whatever your requirements, we are here to ensure your recovery is speedy, comfortable and above all, in the comfort of your own home.

We usually begin planning with yourself, your GP and, or consultant before your admission to hospital. This ensures our staff completely understand your care needs and can agree in advance with you and your consultant or GP, the best plan of care. Following your stay, our nurses and carers will help transfer you home and take care of you in accordance with your pre-agreed care plan.

Palliative Care

At this difficult and sensitive time, we can provide staff to maximise comfort and reassurance for both patient, family and friends. By knowing that trained and qualified staff are always on hand to assist you can.

Wound Care

Our nurses and carers can visit as often as required to change dressings or care for wounds. Because you can arrange visits to suit your schedule, it provide a very convenient alternative to traditional visits.

Injection or Medication Service

Whether you need a one off injection, a course or you require complex medication on a regular long-term basis, we can provide staff who can visit your home at times to suit you to deliver injections, which may enable you to return home from hospital earlier than would otherwise have been safe. Services may include:

• Vitamin injections
• IVF treatment
• Diabetes / Insulin injections
